BILL OF LADING AGREEMENT

BILL OF LADING - FREIGHT BILL AGREEMENT -The undersigned shipper does hereby give authority to Blue Goose Trucking Co. to ACCEPT or REJECT the above described goods with the right of INSPECTION included, and to transport and deliver said goods to the shipper's destination, subject to classifications and tariffs in effect on the date of the issue of this Bill of Lading. It is mutually agreed that the transportation services hereunder are subject to all terms and conditions on Form 1 "UNIFORM MOTOR CARRIER STRAIGHT BILL OF LADING" as pescribed by the PUBLIC UTILITIES COMMISSION OF OREGON in PART 1, RULE 3, of the RULES AND REGULATIONS RELATING TO KEEPING RECORDS AND MAINTAINING ACCOUNTS.

 
Description
 
We Will:

-Include  2 courteous and professionally trained delivery personnel
-Pickup and deliver mattress at specified addresses
-Provide a lift gate equipped vehicle as required
-Inspect product for damage
(You will receive a 2 hour delivery window plus a 1/2 hour courtesy call)

*Must fit in a 14 foot box truck and each item must weigh less than 299 lbs.
*Portland metro areas only 30 miles maximum travel distance, other area extra.

Standard Delivery
•    Includes your mattress being removed from our truck and delivered inside your door.
•    Does not include any setup or un-packaging of the product or removal of any trash associated with the product.
•    Delivery Time frame:
              o     Approximately 2-5 days (same day option available for an extra charge)
•    Cost:
              o     No additional charge

Full Service Delivery
•    Includes delivery of your product, setup of the frame and mattress set in your home, and disposal of all trash associated with the product.
•    Delivery Time frame:
              o     Approximately 2-5 days (same day option available for an extra charge)
•    Cost:
              o     $20

Full Service Delivery + Removal
•    Includes delivery of your product, setup of the frame and mattress set in your home, and disposal of all trash associated with the product.
•    Removal of your old mattress and box spring set
•    Delivery Time frame:
              o     Approximately 2-5 days (same day option available for an extra charge)
•    Cost:
              o     $40
