BILL OF LADING AGREEMENT

BILL OF LADING - FREIGHT BILL AGREEMENT -The undersigned shipper does hereby give authority to Blue Goose Trucking Co. to ACCEPT or REJECT the above described goods with the right of INSPECTION included, and to transport and deliver said goods to the shipper's destination, subject to classifications and tariffs in effect on the date of the issue of this Bill of Lading. It is mutually agreed that the transportation services hereunder are subject to all terms and conditions on Form 1 "UNIFORM MOTOR CARRIER STRAIGHT BILL OF LADING" as pescribed by the PUBLIC UTILITIES COMMISSION OF OREGON in PART 1, RULE 3, of the RULES AND REGULATIONS RELATING TO KEEPING RECORDS AND MAINTAINING ACCOUNTS.

We Will:

-Pickup and drop off 1 item direct to room of your choice
-Provide a lift gate equipped vehicle as required
-Include a 2 man crew to safely handle product
-Unpack and place product at desired location
-Inspect product for damage  
-Connect to power outlet
-Remove all debris associated with delivery
(You will receive a 2 hour delivery window plus a 1/2 hour courtesy call)



*Must fit in a 14 foot box truck and each item must weigh less than 299 lbs.
*Portland metro areas only 30 miles maximum travel distance, other area extra.
