It is mutually agreed that the transportation services hereunder are subject to all terms and conditions on Form 1 "UNIFORM MOTOR CARRIER STRAIGHT BILL OF LADING" as pescribed by the PUBLIC UTILITIES COMMISSION OF OREGON in PART 1, RULE 3, of the RULES AND REGULATIONS RELATING TO KEEPING RECORDS AND MAINTAINING ACCOUNTS.

Description   We Will: -Include 2 courteous and professionally trained delivery personnel -Pickup and deliver mattress at specified addresses -Provide a lift gate equipped vehicle as required -Inspect product for damage -Unpack and place product at desired location -Set up mattress and foundation -Remove all debris associated with delivery -Remove old mattress per your request for recycling (You will receive a 2 hour delivery window plus a 1/2 hour courtesy call) *Must fit in a 14 foot box truck and each item must weigh less than 299 lbs. *Portland metro areas only 30 miles maximum travel distance, other area extra.
